Prescreening

Please review the following instructions related to completing your CDL-15MW CDL Test Waiver/Even Exchange Military: 

This form is to be used by service members to request a waiver of the Commercial Driver’s License Skills and/or Knowledge Tests. Service members who are currently licensed in North Carolina and who are or were employed in a military position requiring the operation of a military motor vehicle equivalent to a Commercial Motor Vehicle within 1 year immediately preceding the date of application may be eligible. The application applies to the issuance of an initial CDL only and cannot be used for upgrades or restriction changes. The form is to be completed by the applicant and the commanding officer and returned to the CDL Section.

Before beginning the application, please know that you will not be able to save your work as you go.  Therefore, please be prepared to provide copies of the following documents for upload:

• Copy of your military ID (Current Service Member)
• Copy of your DD-214 (Veteran)
• Copy of your military driving record
• Copy of your Military Vehicle Operators Identification Card
• 2 year driving record from previous state of issuance if North Carolina license has not been issued for more than 2 years

If applying for the Even Exchange Program (Knowledge and Skills Test Waiver):

• Copy of DOT Medical Certificate (Form MCSA-5876)
• Evidence that, during the 1-year period immediately prior to the application, you were regularly or are currently employed and designated in one of the MOS's that fall under the Even Exchange Program

If you selected an MOS Rating of "Other", you must obtain a commercial learners permit (CLP) for the class of CDL License for which you are applying. For more information concerning a CLP, contact the CDL Team:
